Students interested in having their physical done at school should go to the host school during the time frame listed.  Students must have the parent portion of the physical form completed and signed along with the $30 (cash only) fee at the time of registration.  Students will not be seen by the doctor without the required materials. Physical forms and permission forms may be obtained from the athletic director or home school front office.  Physicals will be limted to the first 100 students and will be done on a first come first served basis.
